These specialists look at the whole person for solutions using non-surgical and non-narcotic rehabilitation-based relearning methods or finding modified ways to return to active life. Outpatient Pulmonary Rehab

If your community doctor has diagnosed you with conditions such as COPD, long COVID, emphysema, or interstitial lung disease, outpatient pulmonary rehabilitation might be the right choice for you. Our 9-week program offers a comprehensive approach, providing education and exercise to enhance your understanding of your respiratory condition.

To take advantage of this program, follow these steps to schedule your outpatient pulmonary rehab:

Request a Referral: Ask your doctor for a referral to our outpatient pulmonary rehabilitation program. This referral is essential to initiate the process and ensure the program is tailored to your needs.

MD faxes history, diagnosis, medication list, pulmonary function test (if applicable) to (203) 294-8705

Our Pulmonary Team will schedule you to come in for evaluation and test prior to your formal enrollment.

Neuropsychology/Counseling

Neuropsychological assessment is often needed when a medical condition or injury has affected a person’s memory or executive functioning skills.


Grieving the loss of one’s physical or mental capabilities following a life-changing event is a natural part of adjusting to life after a serious illness or injury. Counseling with a licensed clinical social worker can help you proceed through your healing journey.


A referral from your doctor is needed for both neuropsychology and counseling.

Wheelchair Assessment

A Wheelchair Assessment Service (WAS) appointment consists of a comprehensive evaluation by certified Assistive Technology Professionals (ATP) to assess the following areas:

• Goals and needs of the equipment user

• Current and previous equipment

• Functional abilities

• Environments in which the equipment will be used in including home, work and school

• Physical assessment including skin and soft tissue, passive range of motion, tone, active movement, pain, posture, balance, sensation and endurance.

Prosthetic and Orthotic Services

Gaylord's board-certified physical medicine and rehabilitation doctors work with patients who require new or revised prosthetics and orthotics. Our team evaluates a patient's strength, gait, balance, coordination, medication, and current prosthetic or orthotic use. New technologies are changing the shape of this field and our clinical staff can introduce patients to the best options available. Whether getting up on a new prosthetic for the first time or interested in new technology, Gaylord can help.

Workers' Compensation Expertise

Workers' Compensation

Gaylord provides coordinated, individually managed, team-based, rehabilitative and complex medical care for injured workers in both inpatient and outpatient areas. Treatment plans are created and led by a board certified medical director with extensive experience in workers' compensation and coordinated by an experienced team of care managers. We provide the injured worker with the latest evidence-based rehabilitation in a healing environment while communicating with the team already involved in the injured workers care.
